Left temporal arachnoid cyst fenestration (endoscopic fenestration)
Little lotta system으로 left temporal arachnoid cyst fenestration을 소개하는 동영상.
좌측 귀 앞쪽으로 3cm 가량의 절개를 하여 Temporalis muscle를 젖힌다. 지름 1cm 이하의 지름의 hole을 만들어 Dura mater 를 자르면 그 바로 밑에 Arachnoid membrane이 확인되는데 여기부터 조심히 Coagulation 을 하여 fenestraion을 하지 않으면 보이지 않는 곳의 혈관 출혈이 발생하여 지혈이 어려울 수 있다.
Cyst 안쪽으로 진입하여 최대한 많은 부위의 membrane을 제거 또는 fenestraion 하는 것이 중요하다. 특히 Sylvian fissure, Cistern 부위의 triangle을 열어주어야, Arachnoid cyst 가 줄어들고 다시 커지지 않는다.
